Notes: Free mushroom permit required and issued by Gifford Pinchot National Forest (Mt. Adams Ranger District Office in Trout Lake, WA. Specimen was collected just east of “Big Tree” on the north side of road 8020. Approximately 3,000 feet. User who defined this location: Richard Bishop
