I found this once in 2009 locally. It took me a year to figure out what it was. I will post my observation shortly.

Even if I’m Italian, I know quite well this species because I collected it in Finland (where it’s quite rare!).
Macroscopical features match perfectly with this taxon! ;-)

…and seems a good match for the H. inocybiformis description and photo on the PNW key council website. Methinks that this mushroom has been misidentified in the past in other collections that I have seen, as well…see todays posting.

It doesn’t look like that to me.
With a dry cap and stem (and cap more or less scaly) I’d suggest this one instead: http://www.svims.ca/...
And it was described from NA by Hesler & Smith from the beginning…
