Notes: Specimen was found growing in soil. The pileus was dirty white to yellow in color and approx 8.5 cm in diameter. It was smooth and broadly convex. On the surface there are holes that may be due to insects eating the specimen. The stipe was a dirty yellow color and approx 6cm in length. The width of the stipe was even throughout. Gills were decurrent and dark tan in color. No volva or annulus was noted.
