Observation 12796: Lepiota cristata (Bolton) P. Kumm.

No herbarium specimen

Notes: Would have called this normally something close to Lepiota rubrotincta, but that has been deprecated to Leucoagaricus. So I suppose this should be called Leucoagaricus also. 3 inches tall, dark brown on the center of the stipe, annulus present, striate margin to the gills, appears to be growing from a bulbous base, perhaps in well-rotted wood.
