Notes: A single specimen growing in mixed broadleaf woods consisting of oak, hickory, maple and beech.
The fruitbody is 14.5 cm tall.
The cap is 5.6 cm in diameter; striations are 1.1 cm long.
The stipe is ~6 mm and 9 mm in diameter at the apex and near the base, respectively.
