Notes: A single robust specimen growing in mixed broadleaf woods consisting of oak, hickory, maple and beech.
The fruitbody is ~ 15cm tall.
The cap is 8.7 × 8.0 cm in diameter; striations are 1.5-1.7 cm long.
The stipe is 7 mm and 15 mm in diameter at the apex and near the base, respectively.
The volval sack is sock-like, 4.5 cm tall; half of it was lost while trying to remove the mushroom from heavy clay soil.
