Does the mushroom have a distinct odor?
I’m not sure if I can help you with a diagnosis, but let’s get some more information on the mushroom so that other folks will start tossing in some ideas.

volva attached to the bulb at the bottom of the stem.
With dead grass attached to the cap, it seems to me that the mushroom may have come up through the ground level detritus with no appreciable volva.
Also, the irregular shapes of an adjacent set of short and long gills to the left of the stem in the bottom photograph is something that rarely if ever happens in Amanita.
