Habitat: well-drained leaf-litter in mixed woods.
Cap: light grayish-tan; diameter 3 cm; flattened-convex but wrinkled & irregular.
Gills: tan, close, maybe free? or maybe collar?
Stalk: colored as cap; slightly fuzzy; no ring, volva, or base bulb; 80-90 mm x ~6 mm but somewhat flattened, widens at cap and toward base.
