pileus marginally striate (striations to about 1 cm), viscid, white with slightly darker buff coloration in center, several small remnants of universal veil tissue, diameter 5.8 cm; volva sac-like; in fir-spruce-aspen habitat

This is probably from section Phalloideae — the volva appears to be saccate, not limbate. The lined cap margin is probably due to the tissue being waterlogged.
