Habitat: three separated individuals on large, rotting stump in mixed woods; ground cover is dry leaf litter.
Cap: color off-white; diameter = 3-5 cm; surface velvety, very slightly sticky.
Gills: close, unequal, decurrent; colored as cap (color of gills in indoor-photo is distorted).
Stipe: short, thick, apex colored as cap and gills, lower gray.
Spores: print color is cream to light yellow; shape is broad oval, 7.6 μm x 5.7 μm.
Flesh: white, soft, crumbly; no particular odor or taste.
