Herbarium specimen reported

Notes: Found on well decayed hardwood log about 18 inches off the ground. Spore print, very light/white. Spores hyaline, smooth. There were two of these, one small one (first picture) cap approx 2.5 cm, and a larger one that had been shredded by animals, but was probably at least 5-6 cm.
