I see this one a lot on alder, but have also seen it on birch.

It is usually found on deciduous logs with beech and birch being favored hosts according to the literature. This was in a mixed woods of planted pines and native broadleaf trees. I would guess this was on elm or maybe tulip (Liriodendron).
