Habitat: a few individuals scatter along the apron of a gravel driveway thru wooded area; well drained.
Cap: white; ~3 cm diameter; nearly flat.
Gills: white; attached; slightly decurrent; crowded with many incomplete gills at margin; wavy.
Stalk: white; fuzzy; no ring; sturdy for specimen size; enlarges toward base.
Spores: white; bean shaped (long oval), see photos.
