At the base of a Tulip Poplar tree attached to buried wood. Images in woods taken with cell phone. PV seemed cortinate when found.
Cap 18 mm. Stipe 4.5 cm long x 4 mm a apex, flexible, hollow. Gills adnate, gray brown.

Kuo’s description seems to match this specimen especially the form of the corrugations.
