Pileus: 4 cm diameter, margin appendiculate, striated, striations 5 mm in from margin, convex, color buff white bruising light red

Stipe: ~7 cm in length, 7-8mm mm at widest part, narrowing towards the apex (5-6 mm), white, abundantly floccose especially at apex; volva white, saccate, firm, unbreaking, 3 cm long

Gills: Free, white

Odor: Earthy

Habitat: Mixed Quercus spp. (nigra, alba, falcata), Liquidambar styraciflua, Acer spp., growing up from leaf litter
