Cap/upper_surface: ca 3-4 cm across; depressed with wavy margin; smooth; dull; slightly tacky; dark brown in center fading to almost white at margin; remnants of lemon-yellow universal veil on cap. Flesh white (no bruising) when cut.
Cap/underside: Gills not attached; some short gills; white
Stipe: Narrowing from apex to ca. 6-7mm, becoming wider at base; bulb base; white to cream fibrils; delicate yellowish annulus; yellow remnants of universal veil on bulb at base.
