Notes:
Pileus: 75 mm, round, plane, slight central depression, dry, off-white background with dark brown coarse fibers, margin rimmed with cottony veil remnants.

Context: :5 mm thick, off-white speckled with beige.

Lamellae: emarginate, crowded, some forking from 1/3 to 1/2 distance from base, thin smooth, white.

Stipe: 54 × 16 mm, solid, attenuated in low, annulus ascending, floccose, white, context off-white speckled with beige.

Spore Print: white.

Spores: 5 – 8.5 × 4.5 – 6 µm, subglobose, rugose, hyaline.

Habitat: directly under oak in mixed deciduous (primarily oak, beech, maple) & hemlock.
