Corner, E. J. H.  1968.  A Monograph of Thelephora.  J. Cramer, Forestburgh, New York.  110p.
After some more digging, it looks like maybe Thelephora americana might be a better fit (because the hymenium is smoothish), but the description is too short to tell for positive and most is still in Latin. Darn Dr. Corner!
